To play for Captain Scott Invitation XI, you’ll need to complete a membership form every new season, to ensure the details we have are current.
Membership forms are completed online and you will be mailed a copy after successfully completing the questionnaire. It shouldn’t take more than 3 – 5 minutes, at most.
If you are 16 or older you’ll need to complete a Senior Membership Form. If you are 15 or younger, a parent or legal guardian will need to complete a Junior Membership Form on your behalf.
Most the information required is simple, straight forward and you’ll be able to supply without any difficulty however, you are required to supply name of your GP, the Name of your GP’s Surgery and a contact number for your GP. People don’t always know this information and might require them to look it up or ask someone else.
